COVID-19 Office Visit Policy and Procedures

 

Patient safety is our top priority. We have implemented a thorough protocol and are closely monitoring CDC guidelines. At this time we are asking patients to book by appointment only.

Appointment/What to expect:

  1. Please arrive on time for your appointment, or we will need to reschedule you (no exceptions) There will be a $150 non refundable cancellation fee if the appointment is cancelled less than 24 hours prior to the scheduled appointment. 

  2. You must be wearing a mask in order to enter the facility. 

  3. Upon entering the waiting room, a temperature check will be done. Anyone above 98.9F, will be asked to reschedule their appointment. 

  4. You will be given a consent form to sign, stating that you have not been exposed to Covid and have no Covid symptoms prior to the appointment.

  5. Hand sanitizer is available to use upon entry and departure.
